Southern Arizona VA Health Case System, Dr. Demetrios Stavrakis
Refractory Pouchitis:
64 y.o. male with history of Ulcerative Colitis s/p IPAA (2007) admitted with progressive diarrhea and abdominal pain. Pouchoscopy and imaging suggestive of de novo Crohn's disease of the pouch with involvement of afferent limb of the pouch.

Remission of UC:
70 y.o. woman presents for 1-year follow-up of pan-ulcerative colitis diagnosed in 2013 who has been in clinical remission on mesalamine 2.4g/day since diagnosis. She takes no other medications and is able to perform ADLs without assistance.

Intestinal Behcet's:
An 83-year-old man with mouth sores, hypotension, fever, and unusual colonic lesions.
Clinical Vignette:
Southern Arizona VA Health Care System, Dr. Michael Gavin
54 yo male with IDA and reported history of Crohn's Colitis found on recent colonoscopy to have segmental colitis associated with diverticulosis (mild-moderate UC-like pattern).

32yoF presents with severe acute UC during pregnancy

COVID-19:
39 y/o F with UC on tofacitinib hospitalized with COVID-19 and respiratory failure now being discharged.

Young man with UC and history of LGD in the distal colon who elected surveillance with chromoendoscopy over colectomy. Now with likely advanced neoplasia.

21-year-old male with a complicated medical history related to Kimura 's disease including pituitary
dwarfism, ESRD 2/2 FSGS s/p DDKT (01/2010) on tacrolimus/sirolimus, ITP (on chronic prednisone), with
severe pan-UC with medicine induced vs. autoimmune mediated severe neutropenia

Late-Onset IBD:
82 year old male with development of colitis followed by rectal stricture after cardiac surgery.
Case 2:
SAVAHCS, Dr. Eugene Verkhovsky
Strictures:
71 yo. male with history of Crohn's Disease (Ileal/Fibro-stenosing phenotype) diagnosis 1971 s/p distal ileal resection with ileocolonic anastomosis (1984) found to have anastomotic stricture (2010) reports intermittent right -sided abdominal pain with associated episodes of obstipation over the past several months. Small Bowel Follow-Through demonstrates 2-2.5 cm stricture at ileocolonic anastomosis.
