Drug induced fevers in ulcerative colitis:
28 y.o. with fevers with clinical improvement in ulcerative colitis symptoms while on mesalamine, prednisone, and adalimumab.

Clinical Vignette:
Oregon Health & Science University, Dr. Anil Sharma

Cancer:
26 year old man with long-standing indeterminate colitis on golimumab who was transferred to OSHU for further work-up and management of newly diagnosed adenocarcinoma (presumed colorectal primary) with hepatic metastases.

Pouch Dysplasia:
73 y.o. male with pmh of 40 years of extensive ulcerative colitis and unresectable rectal adenoma w/HGD s/p laparoscopic total proctocolectomy with ileostomy, reversal, and IPAA creation found to have dysplasia on random biopsy of the pouch.

Therapy Deescalation:
23 yo woman with h/o left-sided ulcerative colitis on adalimumab, azathioprine and mesalamine in endoscopic and histologic remission for 3 years. Do you de-escalate therapy and how?

Strictures:
29 y/o F with history of medically refractory stricturing ileocolonic Crohn's disease (dx 2007) currently pending transition to AZA + GLM who presents for continued follow-up and consideration for stem cell transplant.


Clinical Vignette:
Palo Alto Medical Foundation, Dr. Ryan McConnell

Pseudopolyps:
20-year-old man with severe Crohn's colitis in steroid-free clinical remission on vedolizumab every 4 weeks, extensive pseudo polyposis, and elevated calprotectin. Should he change therapies? Should he eventually have a colectomy?

Main Case:
Southern Arizona VA Health Case System, Dr. Demetrios Stavrakis

Refractory Pouchitis:
64 y.o. male with history of Ulcerative Colitis s/p IPAA (2007) admitted with progressive diarrhea and abdominal pain. Pouchoscopy and imaging suggestive of de novo Crohn's disease of the pouch with involvement of afferent limb of the pouch.

Remission of UC:
70 y.o. woman presents for 1-year follow-up of pan-ulcerative colitis diagnosed in 2013 who has been in clinical remission on mesalamine 2.4g/day since diagnosis. She takes no other medications and is able to perform ADLs without assistance.

Intestinal Behcet's:
An 83-year-old man with mouth sores, hypotension, fever, and unusual colonic lesions.

54 yo male with IDA and reported history of Crohn's Colitis found on recent colonoscopy to have segmental colitis associated with diverticulosis (mild-moderate UC-like pattern).
